We are looking for a Care Assessor to join our family like team. Where you’ll provide life-changing care and support that enables vulnerable adults of all ages to live fulfilling lives in our Supported Living location. 

As an Assessor, your role will be diverse to each client. You will be responsible for establishing a professional client relationship, undertaking new client assessments and writing person centred care plans. You will be the first face of the business and will need to build and maintain relationships with clients and their loved ones/representatives, as well as colleagues, ensuring staff have effective supervisions, personable support, and their training remains up to date.

Our Assessors must have exceptional time management skills, with the ability to prioritise and organise their own workload. A high level of attention to detail is needed, along with a passion for high quality care standards also being critical for this position.

Our usual working shifts are:

  • Mornings 7am-2pm
  • Evenings 2pm-10pm
  • Alternative weekends

This is a full time position with mixed hours of both Care Assessor and Care Assistant roles. Approximately 20 hours Care Assessor role and 20 hours Care Assistant role.

Job Description:

  • Writing person centred care plans
  • Carrying out new client assessments and risk assessments
  • Undertaking Mental Capacity Assessments and completing Best Interest Decision paperwork
  • Leading on client reviews and undertaking any resulting actions
  • Medication reviews and audits
  • Monitoring the quality of care delivered through spot checks
  • Our care assistants are responsible for assisting service users with personal care such as washing, bathing and toilet arrangements.
  • Helping service users overcome any mobility problems and other physical disabilities, including helping in the use and care of aids and personal equipment.
  • Domestic tasks such as making and changing beds, tidying rooms, light cleaning, laundry and emptying commodes
  • Help with eating and drinking by preparing meals, snacks and drinks
  • Support with administrating medication.
  • Contribute as needed to a person’s end-of-life care

Person Profile:

  • Experience in mental health would be an advantage
  • Minimum 1-year Domiciliary Care experience
  • Previous experience of writing care plans -desirable
  • Experience of conducting risk assessments desirable
