We're looking for a motivated, organised and compassionate Care Coordinator to join our Gloucestershire team. Whether you're an experienced Care Coordinator or a Senior Care Worker, Field Care Supervisor or Assessor ready to take the next step in your career, we'd love to hear from you.

No two days are ever the same. You'll be coordinating care, supporting our dedicated team, building relationships with clients and families, and helping us continue to deliver outstanding domiciliary care across Gloucestershire.

What You'll Be Doing

As our Care Coordinator, you'll play a key role in the day-to-day running of the service by:

  • Planning and scheduling care visits to ensure safe and efficient delivery.
  • Building and maintaining effective staff rotas.
  • Carrying out care assessments and customer reviews.
  • Supporting, supervising and developing our care staff through supervisions, spot checks and competency assessments.
  • Assisting with recruitment, including interviewing prospective care staff.
  • Supporting medication processes, including MAR chart audits and medication transcribing.
  • Completing ECM reconciliations accurately.
  • Acting as a key point of contact for clients, families and care staff.
  • Helping develop training materials and maintaining training records through Nourish.
  • Delivering the Care Certificate following completion of Train the Trainer.
  • Taking part in the on-call rota (additional payment provided).
  • Providing general office support where required.

What We're Looking For

You'll be someone who enjoys working in a fast-paced environment and takes pride in delivering excellent customer service.

You'll also have:

  • Previous experience within Health & Social Care (essential).
  • Experience within domiciliary care is desirable.
  • Excellent organisational and communication skills.
  • The ability to manage changing priorities and solve problems calmly.
  • A positive, supportive and team-focused attitude.
  • A full UK driving licence and access to your own vehicle.
